PHOTO RELEASE/PERMISSION FORM

This form grants Guided By Grace Images permission to use images from your event for the sole purpose of advertising and promotion. This form is OPTIONAL.

Full Permission - I grant Guided By Grace Images, FULL PERMISSION to use any and all images taken at this event for the sole purpose of advertising and promotion including such media as photographic prints, digital albums, CD, DVD, and/or website viewing including the SAMPLE IMAGE GALLERY. I understand that the sample image gallery is available for viewing by any person(s) who may access the website.

Limited Permission - I grant Guided By Grace Images, LIMITED PERMISSION to use any and all images taken at this event for the sole purpose of advertising and promotion including such media as photographic prints, digital albums, CD, DVD, and/or website viewing. I do not wish to have my images available in the SAMPLE IMAGE GALLERY. I wish to have all images which are posted on the website protected by a pass code. I grant Guided By Grace Images permission to give this pass code only to attendees of the event that I have hired them to photograph.
